Folliculotropic mycosis fungoides of the scalp: A case report

Abstract: Folliculotropic Mycosis Fungoides (FMF) is a
rare but distinct variant of mycosis fungoides, characterized by predominant
follicular involvement and a typically more aggressive course. We report the
case of a 65-year-old woman with a four-year history of
scarring alopecia
of the scalp associated with intense pruritus. Clinical and dermoscopic
findings raised suspicion of a lymphoproliferative disorder. Histopathologic
examination and immunohistochemistry confirmed the diagnosis of FMF (CD3+,
CD4+, CD5+, CD7?, CD30?). FMF can mimic various dermatoses, leading to
diagnostic delays. Management relies primarily on PUVA phototherapy, combined
with systemic treatments. Early diagnosis is crucial due to the potentially
aggressive behavior of this subtype.
